One Fear BJJ at Attawheed Islamic Center Liability Waiver and Release of Claims
Assumption of Risk and Release of Liability
I, the undersigned participant (or parent/guardian if participant is a minor), acknowledge that I have voluntarily chosen to participate in Brazilian Jiu-Jitsu (BJJ) training, classes, and related activities (the “Activities”) at One Fear BJJ at Attawheed Islamic Center in Allegheny County Pennsylvania. I understand and agree to the following terms:
1. Acknowledgement of Risks: I understand that the Activities involve strenuous physical exertion, and include the risk of personal injury, including but not limited to bruising, sprains, broken bones, joint injuries, muscle strain, spinal injuries, concussions, or in rare cases, severe injury or death. I acknowledge that these risks may be due to my own actions, the actions or negligence of others, the nature of BJJ, and/or the conditions of the facility and equipment.
2. Assumption of Risk: I willingly assume full responsibility for the risks associated with participation in the Activities, including any injury, harm, or loss I may suffer while participating in BJJ classes, training, or events.
3. Release of Liability: In consideration of being allowed to participate in the Activities, I, on behalf of myself, my heirs, personal representatives, and assigns, hereby release, discharge, and hold harmless One Fear BJJ, Attawheed Islamic Center, and their respective instructors, employees, representatives, volunteers, affiliates, and agents (collectively, the “Released Parties”) from any and all claims, demands, actions, causes of action, liabilities, damages, losses, and expenses (including attorneys' fees) arising out of or in any way related to my participation in the Activities, including those arising from the Released Parties’ negligence.
4. Medical Fitness and Consent to Treatment: I represent that I am in good physical condition and do not suffer from any condition or injury that may affect my ability to safely participate in BJJ classes. I understand that BJJ is a contact sport and that injuries are possible. In case of emergency, I consent to the administration of first aid and medical treatment by an appropriate medical professional and assume all responsibility for any costs associated with treatment.
5. Code of Conduct and Acknowledgement of Policies: I agree to follow all rules, policies, and instructions given by One Fear BJJ instructors and staff. I understand that failure to adhere to these rules may result in my suspension or dismissal from the Activities without refund.
6. Photo and Video Release: I give permission for my image and likeness to be used in promotional materials, including photographs and videos taken during classes or events, unless I notify One Fear BJJ in writing that I do not wish to grant this permission.
7. Governing Law and Severability: This waiver and release shall be governed by the laws of the Allegheny County Pennsylvania. If any provision is found to be unenforceable, the remaining terms shall remain in full effect.
Acknowledgment and Signature: By checking the box on the registration form, I acknowledge that I have read, understand, and agree to all terms of this waiver and release of liability. I affirm that I am accepting this waiver voluntarily and with full understanding of its contents.
